About The Job – Your RoleClinical Pharmacy Practitioner will work to improve patient care, reduce pharmaceutical spending and act as a resource for providers, nurses and pharmacists when challenging pharmaceutical questions and circumstances arise that require significant time and research to resolve. As the Clinical Pharmacy Practitioner, you will be assigned to take part in a Collaborative Therapy Management (CTM) program as a mid‑level practitioner in which the pharmacist participates in the management of drug therapy, directly providing care to patients.
The Clinical Pharmacy Practitioner will be training nursing staff and providers on pharmacy software systems, assist in maintaining a working formulary with vigilant ongoing review of new pharmaceuticals and the value they could present to our adults in custody (AICs), and provide regular pharmaceutically aligned meetings aimed at improving knowledge base of Health Services staff.
You will review patient treatments in the aspects of polypharmacy, medication therapy management (MTM), assist in treatment plans and attend Therapeutic Level of Care (TLC) committees to act as a drug expert when discussing pharmaceutical treatment regimens. Provide advice and consultation to physicians and other direct care staff in the treatment of AICs, including providing information to AICs to increase their understanding and compliance to ordered drug regimens.
You will engage in discussion with outside providers and specialists to procure Specialized Pharmaceuticals that offer good patient care while having the least financial impact.
This position is represented by American Federation of State, County, and Municipal Employees (AFSCME). This position may be eligible for Police & Fire (P&F) benefits through Public Employees Retirement System (PERS), as defined by statute.
Minimum QualificationsA current Pharmacist's license and must be able to obtain an Oregon pharmacist license issued from Oregon State Board of Pharmacy prior to hire, AND three years of professional experience related to the class concept.

- Graduation from an accredited School of Pharmacy
- State of Oregon Pharmacist License, upon hire
- Computer experience – MS Office Suite, Pyxis
- Clinical experience in application of evidence-based guidelines
- Board Certification (BCPS, BCACP or similar)
Preference will be given to those with Medication Therapy Management (MTM) experience and experience working in a correctional facility or jail.
