Drawing on your expertise in physical assessment, therapeutic intervention, and patient education, you’ll help individuals regain mobility, restore confidence, and enhance their overall quality of life.

In this role, you’ll collaborate closely with physicians and the interdisciplinary home health team to develop and implement individualized care plans that support recovery, reduce the risk of hospital readmission, and promote long-term wellness. You’ll be responsible for evaluating, planning, directing, and administering physical therapy services as prescribed by a licensed physician, nurse practitioner, or physician assistant.

As a valued member of the home health team, you will report directly to the Home Health Director. Enjoy the benefits of a flexible schedule, a supportive and collaborative work environment, and no weekend or holiday shifts.
