As the resource RN you will assist case managers of the dedicated team by doing supplemental patient visits to assess and reassess the nursing needs of the patient. You will provide coverage for the case manager during their absence. This position will also assist in educating families and provide mentor training for newer nurses on the team as well as assisting in senior living relationships.
This position has the expectations of working Monday - Friday, 8:00am-5:00pm. The Resource Nurse is assigned to a dedicated team within the Capstone service area.
Requirements
Candidates must meet the following requirements to be considered:
- Graduate from an accredited school of nursing
- Currently licensed as a registered nurse to practice in the state.
- Minimum 2 years of hospice experience as a registered nurse
- Active patient contact within past three years
- Experience in nursing documentation using a dedicated EMR
- Experience with pain and symptom management to include complex symptoms diseases.
- Proven experience with the end-of-life process.
- Proficient in clinical documentation and communication.
- Ability to work independently, make accurate, and at times, quick judgments
- Ability to respond appropriately to crisis outside of a hospital setting.
- Ability to provide clear, compassionate communication.
- Ability to manage grief and maintaining boundaries despite frequent loss.
- Ability to care of patients in their private residence and senior living locations.
- Acceptance of and adaptability to different social, racial, cultural and religious modes.
