• Assess and monitor the health status of patients with chronic conditions
• Develop individualized care plans in collaboration with multidisciplinary teams
• Educate patients and families on disease management and self-care practices
• Provide emotional support and guidance to patients throughout their treatment journey
• Administer medications and treatments as prescribed by physicians
• Maintain accurate patient records and documentation

• Active RN license in the state of Massachusetts
• Experience in managing chronic illnesses preferred
• Strong interpersonal skills and empathy towards patients
• Ability to work collaboratively within a healthcare team
• Excellent organizational skills and attention to detail
