As a Physical Therapist, you will:

• Provide direct rehab care in physical therapy to patients upon physician referral.
• Adhere to all applicable laws, regulations, and Life Care standards.

• Graduate of an accredited program in physical therapy (BSPT, MSPT, or DPT). If graduated in 2003 or later, must have MSPT or DPT.
• Currently registered/licensed in the applicable state, with an active license in good standing throughout employment.
• One (1) year of experience in post-acute care or a related setting is preferred.
• CPR certification upon hire or to be obtained during orientation, with ongoing maintenance required.

• Participate in community health initiatives and projects.
• Communicate effectively with patients, families, and support departments to meet patient needs.
• Supervise therapy staff and maintain patient documentation.
• Implement and interpret departmental programs, goals, and policies.
• Exhibit excellent customer service and maintain professional relationships.
• Ensure confidentiality of all proprietary information.
• Adhere to Life Care’s Code of Conduct and complete necessary compliance training.

• Follow and modify physical therapy treatment plans for patients.
• Establish and assess realistic, measurable, and functional goals.
• Oversee and evaluate care provided by PTAs, Rehab Aides, and students.
• Chart patient progress accurately and in a timely manner.
• Utilize therapy software effectively.
• Demonstrate dependable attendance and a positive attitude.
• Engage in interdisciplinary teamwork to enhance patient care.
• Maintain physical stamina to perform various tasks throughout the day.
• Communicate fluently in English, both verbally and in writing.
