Position SummaryAs a Physical Therapist, you will:
- Provide direct rehab care in physical therapy to patients upon physician referral.
- Adhere to all applicable laws, regulations, and Life Care standards.
- Graduate of an accredited program in physical therapy (BSPT, MSPT, or DPT). If graduated in 2003 or later, must have MSPT or DPT.
- Currently registered/licensed in the applicable state, with an active license in good standing throughout employment.
- One (1) year of experience in post-acute care or a related setting is preferred.
- CPR certification upon hire or to be obtained during orientation, with ongoing maintenance required.
- Participate in community health initiatives and projects.
- Communicate effectively with patients, families, and support departments to meet patient needs.
- Supervise therapy staff and maintain patient documentation.
- Implement and interpret departmental programs, goals, and policies.
- Exhibit excellent customer service and maintain professional relationships.
- Ensure confidentiality of all proprietary information.
- Adhere to Life Care’s Code of Conduct and complete necessary compliance training.
- Follow and modify physical therapy treatment plans for patients.
- Establish and assess realistic, measurable, and functional goals.
- Oversee and evaluate care provided by PTAs, Rehab Aides, and students.
- Chart patient progress accurately and in a timely manner.
- Utilize therapy software effectively.
- Demonstrate dependable attendance and a positive attitude.
- Engage in interdisciplinary teamwork to enhance patient care.
- Maintain physical stamina to perform various tasks throughout the day.
- Communicate fluently in English, both verbally and in writing.
