CLASSIFICATION: Full-Time – 37.5 hours/week
ANNUAL SALARY: $76,000 – $80,000 (signing bonus)
JOB SUMMARYThe Nurse‑Family Partnership (NFP) Nurse Home Visitor (NHV) is responsible for providing comprehensive nursing services to parents and their families eligible for the NFP program. Nurse home visitors maintain the highest standards in clinical nursing practice, case management and care coordination, adherence to the NFP model, and to policies, procedures, guidelines, and standards of NFP and of The Partnership for Maternal and Child Health of Northern New Jersey.
The NHV provides home visits to parents eligible for the NFP Programs (low‑income, first‑time birthing parents and their families, from early pregnancy up to 2 years of age), adheres to nursing process and the NFP model of home visitation. Nurse home visitors report to the NFP Nurse Supervisor.
- Provide in‑person home visitation to eligible families and carry a caseload of up to 25 clients in accordance with the NFP Model Elements and guidelines.
- Complete all required NFP education and nurse‑home visitor competency requirements, including PIPE:
Partners in Parenting Education, and ongoing professional development and webinar training. - Develop therapeutic relationships and utilize concepts of reflection and motivational interviewing with families in a home visiting environment.
- Assess the physical, emotional, social, and environmental needs of parents and their families as they relate to the NFP domains.
- Assist parents and their families in establishing goals and outcomes; provide education, support, and referral resources in attaining goals; evaluate clients’ progress and plan home visits in accordance with clients’ goals and NFP outcomes.
- Consult and collaborate with other professionals involved in providing services to families.
- Meet with the NFP Nurse Supervisor weekly for clinical supervision and schedule joint home‑visit every four months.
- Attend and participate in weekly team meetings and case conferences.
- Provide information to support staff for timely and accurate data input to the NFP database.
- Participate in review and analysis of reports for achievements and areas for improvement.
- Maintain confidentiality and adhere to the ANA and NJ nursing process and standards, code of ethics, HIPAA confidentiality standards.
- Develop and maintain community relationships to support client referrals.
- Handle other assignments as requested.
- Associate’s degree in nursing required;
Bachelor’s degree in nursing preferred. - Current NJ RN license in good standing required.
- BLS certification required.
- Two years’ experience in maternal/child health, public health, home visiting, or mental/behavioral nursing preferred.
- Excellent written and verbal communication skills.
- Ability to work independently and manage client visit schedules to meet program visit requirements.
- Strong documentation skills to ensure data entry is properly completed in a timely manner.
- Strong organizational skills and ability to handle multiple priorities.
- Basic computer skills and proficiency with Microsoft 365.
- Valid driver’s license and reliable transportation required for regional travel.
