Social Worker LSW / CSW

Lincoln Park Care Center is looking to hire a Social Worker LSW / CSW to support residents and staff in providing exceptional care and a happy environment. This role focuses on emotional support for residents, care planning, interdisciplinary collaboration, discharge planning, and educational outreach.

• Assists residents with emotional adjustment to facility.
• Interviews new residents, gathers social history, and completes assessments.
• Completes the social services portion of the MDS, care planning and baseline data.
• Refers residents to psychological services as needed.
• Schedules and attends interdisciplinary care plan meetings, utilization review, and morning meetings.
• Writes summaries, progress notes, and discharge summaries as directed.
• Conducts pre-discharge evaluations and assists in discharge planning, following up after discharge.
• Writes discharge letters and other required documentation.
• Builds professional relationships with each resident and family.
• Educates residents and representatives on advanced directives.
• Adheres to all Social Services regulations.

• Bachelor’s Degree in Social Work from an accredited college.
• Must possess a CSW and/or LSW license.
• One to three years of professional experience or equivalent combination of education and experience.
