Job Type/Status: Full-Time;
Evening and weekend work may be required to meet the needs of staff and families.
Salary: $50,000 to $55,000 per year
POSITION SUMMARYThe position of Care Manager serves an important role in our agency's mission to prevent child abuse. As a member of our team, the Care Manager will work alongside the Parent Aide to connect families to needed resources in the community and provide individual support to ensure successful service linkage. The Care Manager will act as a resource broker, by building a relationship with the community, collecting community-based resources, and linking families to supports that will enhance their protective factors.
DUTIESAND RESPONSIBILITIES Program Coordination and Community Engagement
Collaborate with Parent Aide and Program Manager/Supervisor to identify immediate service needs for families.
Develop and maintain relationships with a diverse array of community programs to effectively meet the varied needs of families.
Provide care coordination, advocacy, and follow‑up for families in need.
Ensure the successful linkage to community resources by providing transportation, if needed.
Resource Management and CollaborationCollaborate with families, Division of Child Protection and Permanency (DCP&P), and community stakeholders to identify viable resource options.
Identify gaps and barriers to services, advocating for families as needed.
Refer families to services in a timely manner, prioritizing those requiring a plan of safe care.
Participate in advisory councils/boards within their local community/area of service.
Documentation and ReportingComplete required documentation, forms, and produce statistical reports.
Submit monthly/quarterly progress reports focusing on process and outcome indicators.
Participate in quality improvement and evaluation activities to enhance service delivery.
Development of Professional SelfParticipate in agency activities, required meetings, and trainings as mandated by the agency and Department of Children & Families.
Regularly research and document community resources including services related to health, housing, education, employment, and social supports.
Share updated resource information with the Parent Aide and Program Manager/Supervisor.
REQUIREMENTSBachelor's degree in Psychology, Social Work or equivalent experience within the child welfare system preferred.
Experienced in parent/family support, maternal‑child health, or adolescent mental health.
Demonstrates knowledge and experience in care coordination for families and community stakeholders.
Embedded in the communities that are being served.
Demonstrates compassion and empathy in relating to clients with diverse needs.
Cultural competency and ability to work with diverse populations.
Translates complex concepts into easily understandable language for families.
Shows awareness of cultural diversity and its impact on service planning and provision.
Valid driver's license and reliable transportation.
