$47.00 - $56.16 /hr
Full-time Day and PRN available for all shifts
$4.00 shift differential for Noc

In addition to rehabilitation services, Avalon Health and Rehabilitation Center is designed to care for your medical conditions through personalized 24-hour skilled nursing care provided by licensed nurses and therapists.

Minimum pay $47.00. Maximum pay - $56.16. Placement within the wage range for this position is determined by years of experience, qualifications, and demonstrated ability to perform the essential functions.
Responsibilities- Provide direct nursing care to residents in accordance with care plans, facility policies, and regulatory guidelines.
- Administer medications and treatments accurately and timely.
- Monitor residents’ conditions and report significant changes to physicians and other healthcare team members.
- Conduct assessments and document patient progress, symptoms, and vital signs.
- Coordinate with interdisciplinary teams to develop and implement individualized care plans.
- Supervise and support LPNs, CNAs, and other nursing staff to ensure high-quality resident care.
- Respond promptly to emergencies and provide appropriate interventions.
- Educate residents and families on health conditions, care plans, and post-discharge instructions.
- Maintain accurate and timely clinical documentation in electronic medical records (EMR).
- Ensure compliance with infection control protocols and health and safety regulations.
- Promote dignity, independence, and overall well-being of residents.
- A current, active license to practice as a Registered Nurse (RN) in the state of practice.
- Demonstrate RN clinical assessment skills at the level necessary to meet the job requirements.
- Recent clinical experience, education, or specialty skills specific to geriatrics.
- Understanding that state and Federal rules and regulations govern practices within the facility.
- Ongoing pursuit of Continuing Education Credits in clinical subjects, management, personal growth and development.
- Must pass criminal background investigation, reference inquiry, and vaccine mandate attestation requirements or documentation for medical or religious exemption.
