POSITION SUMMARYWellness Coordinator is responsible for overseeing the collaborative process in the Community Partnership School related to facilitating and tracking the school health care program’s referral network and processes. Responsible for coordinating efforts that will benefit the school’s needs with these programs. Additionally, this role coordinates efforts in the evaluation process, including survey of students, parents, and staff for progress monitoring of effective programming.
WHATYOU WILL DO
- Serve as the primary point of contact between the Community Partnership School administration and the medical clinic
- Act as the entry point for students, parents, and community members seeking resources and supp
- Coordinate and oversee all medical clinic referrals, including health, dental, vision, and mental health service
- Track, document, and follow up on all referrals made within and outside the schoo
- Establish and maintain referral relationships with external agencies and service provider
- Participate in intervention team meetings and provide input on behavioral interventions and referrals
- Record professional assessments, treatment plans, and progress notes using the data management syste
- Ensure accurate documentation, maintain required files and records, and prepare program statistics and report
- Participate in crisis intervention as needed
- Implement and oversee eligibility determination, billing, and reimbursement processes
- Participate in quality improvement activities and initiative
- Submit all required documentation within established timeframe
- Maintain familiarity with the public-school system and the communities served
- Stay current with and implement Pasco County School District policies and procedures
- Serve as a liaison to local schools by attending SAC meetings
- Chairs the Student Leadership Council
Minimum of three years’ experience in education and/or community outreach. Experience serving as an advocate for children and parents and preferably in an educational setting, building community relationships and convening community resource programs. Possess knowledge of student support programs, evidence based interventions and community and school-based prevention services. Two-year college degree. Valid Florida driver’s license with reliable transportation and insurance.
COMMUNICATION SKILLSExcellent communications skills (verbal and written). Ability to analyze data, identifying problems and trends. Excellent organizational and interpersonal skills and strong ability to network and engage others. Ability to handle confidential information appropriately. Flexibility to include ability to work evenings and weekends when needed.
COMPUTER SKILLSBasic computer skills that allow for creating Word and Excel documents are required.
