Inpatient Case Manager​/Licensed Vocational Nurse - Hybrid

Health Care Support is seeking a Inpatient Case Manager to join a mission-driven team in the Pasadena/Arcadia, CA area. This full-time, salaried role offers autonomy in the field and remotely.

• Arcadia, CA

• Monday – Friday, 8:00 AM – 5:00 PM
• Hybrid - 2-3 days on site at hospital & remaining days remote
• No nights, No on-call, and no weekends!

• $31-$38 per hour

• Ensure compliance with Case Management policies and health plan Utilization Management standards, including accurate medical necessity and level-of-care reviews using Milliman Care Guidelines.
• Coordinate inpatient case management activities by reviewing medical records, conducting initial and concurrent reviews, collaborating with providers and discharge planners, and supporting timely, appropriate discharge planning.
• Communicate determinations and case updates with medical directors, health plans, providers, and leadership while maintaining strong professional relationships and on-site hospital availability.

• Active Licensed Vocational Nurse (LVN) license in California
• Experience using Microsoft applications such as Word, Excel and Outlook
• At least two (2) years of clinical experience as an LVN
