Job Summary

As the leader of the health care team, provides professional nursing care, utilizing the nursing process in accordance with established standards of care, policies, and procedures. Demonstrates performance consistent with the Medical Center Vision, Mission, and Strategic Plan of the organization. Remains flexible to changing systems; is expected to demonstrate quality and effectiveness in work habits and clinical practice; and treats co-workers, patients, families, and all members of the health care team with dignity and respect.

Responsibilities
  • The Registered Nurse demonstrates proficiency by exhibiting the following skills, competencies, and behaviors:

  • Nursing Process:
  • Develops and/or contributes to the individualized plan of care that reflects assessment, planning, implementation, and evaluation of outcomes.
  • Ensures plan shows multidisciplinary planning, consultation, and education; reflects admission or outpatient data, ongoing findings, age-appropriate care, cultural needs, and appropriate acuity.
  • Ensures plan is discussed with patient, family/significant others, and completed in a timely manner.
  • Monitors patient progress based on the plan and revises the plan as needed based on condition and progress evaluation.
  • Ensures care meets standards of practice and supports discharge planning and patient/family education.
  • Ensures patient safety in medications and procedures, fall prevention, decubitus prevention, and infection prevention.
  • Ensures optimal pain control and patient comfort; addresses patient anxieties, fears, or concerns.
  • Ensures patient understanding of medication purposes, side effects, and administration instructions in-hospital and at discharge.
  • Patient Care

    Experience:
  • Practices customer service standards as defined by the Service Area, Medical Center, and specified department.
  • Promptly answers call lights, alarms, and patient requests; makes appropriate referrals and facilitates resource use.
  • Maintains patient confidentiality; ensures a clean, orderly, and functional work environment.
  • Treats all families of patients with courtesy, respect, kindness, and compassion; actively listens to patient and family needs and ensures understanding.
  • Provides a patient care experience that exceeds members' expectations.

  • Viewed by others as an effective team member who is flexible, cooperative, and willing to assist others.

  • Fiscal Responsibility:
  • Organizes work to minimize overtime; identifies and assists in systems improvements; utilizes payroll and non-payroll resources to their maximum potential.
Experience

Basic Qualifications:

  • Minimum one (1) year recent (within the last three years) full-time equivalent experience in a licensed Home Health, Palliative Care and/or Hospice agency as an RN/PHN.

License, Certification, Registration
  • Registered Nurse License (California)
  • Driver's License (California)
  • Basic Life Support
Additional Requirements
  • Clean Driving Record for the last 2 years.
  • Proof of automobile insurance as required by law.

Notes
  • Sunday-Saturday, alternating weekends
  • If ten (10) hour option chosen, shift will be 8:30 am-7:00 pm, back up 8-hour shift will be 8:30 am to 5:00 pm.
