The Registered Nurse - Case Manager is responsible for planning, developing, assessing, and evaluating care provided to patients.
This role involves collaboration with multidisciplinary healthcare teams, patients, and families to create, implement, and document individualized care plans.
The RN Case Manager ensures continuity of care, efficient resource utilization, and compliance with regulatory requirements.
Key Responsibilities- Develop, implement, and document individualized care plans in collaboration with physicians, healthcare teams, and patients/families.
- Assess high‑risk patients in need of post‑hospital care and create discharge plans to meet identified needs.
- Communicate care plans and discharge instructions to all involved parties, including community agencies.
- Monitor and coordinate the patient’s hospital stay, ensuring timely delivery of essential services.
- Facilitate interdisciplinary continuity of care, including utilization management, discharge planning, and coordination of external services as needed.
- Conduct daily clinical reviews for utilization and quality management across outpatient, emergency room, inpatient, and referral facilities.
- Act as a liaison between inpatient and referral facilities/agencies, coordinating patient transfers as needed.
- Educate healthcare team members on their roles in discharge planning and appropriate resource use.
- Provide patients with education to support their discharge and address psychological aspects of acute and chronic illness.
- Participate in case review and report quality, risk, and patient safety incidents.
- Analyze utilization patterns, identify trends or resource issues, and contribute to data collection for studies and utilization monitoring.
- Coordinate and participate in care planning rounds, patient-family conferences, and committee projects as required.
- Current RN license
- Must have at least 2 years of recent experience in the specialty within the past 2 years
- Knowledge of discharge planning and care coordination practices
