In a manner consistent and supportive of our values, the Primary Care Provider is responsible for delivering high quality health care within Fast Pace Health’s scope of services while achieving optimum patient satisfaction. The Primary Care Provider is the first point of contact for preventative care and continues to provide comprehensive care focused on long‑term health and well‑being.
- Conduct thorough physical examinations and assessments for patients of all ages, including children aged 3 years and up.
- Perform chronic care management for patients with conditions such as hypertension, diabetes, and asthma.
- Administer annual wellness visits for adults and pediatrics, including vaccinations and preventive screenings.
- Order, interpret, and evaluate diagnostic tests to diagnose and assess patients’ clinical problems and healthcare needs.
- Collaborate with physicians and other healthcare professionals to develop comprehensive patient care plans.
- Prescribe medications, recommend treatments, and provide written home‑going instructions.
- Refer patients to specialists or other healthcare resources as needed.
- Educate patients and their families on health maintenance and preventive care.
- Promote patient engagement, health literacy, and shared decision‑making.
- Participate in quality improvement initiatives and value‑based care programs.
- Emphasize patient‑centered care, continuity of care, and build strong patient‑provider relationships.
Ability to provide quality care and meet patient volume goals, with an expectation of +18 daily on average.
- Lead clinic staff (e.g., X‑Ray Technologists, Nurses, Front Office Specialists) in delivering excellent patient care.
- Provide guidance to ensure quality professional services and high patient satisfaction.
- Discuss and review patients’ medical history, symptoms, allergies, and current medications to formulate accurate diagnoses.
- Actively engage with clinical leadership and elevate concerns to management when appropriate.
- Ensure accurate completion of patient charts in a timely manner and forward charts as needed daily.
- Work efficiently in a fast‑paced autonomous environment.
- Dedicate to evidence‑based medicine and stay current with medical advancements.
- Implement clinical protocols as outlined by CMO and supervising physicians.
- Perform responsibilities within standard NP protocols.
- Analyze and interpret patients’ histories, physical findings, and diagnostic information to develop diagnoses.
- Deliver excellent patient care with attention to patient‑centric care, safety, cost, and reliable accuracy.
- Ensure compliance with Federal and State regulations, including HIPAA.
- Complete mandatory compliance training and maintain company quality standards.
