Overview

Under the general supervision of the Division Director of Care Coordination, you will develop, implement, and manage day-to-day operations of WHS’s Care Coordination Programs. Our Care Coordination services include assessment, service planning, linkage/referral, monitoring, and follow-up activities, transportation, and reporting services.

Education and/or Experience
  • Bachelor's degree or non‑licensed master level clinician required, with either degree in behavioral health or human services preferred (psychology, social work, marriage and family therapy, counseling).
  • A minimum of 3 years of leadership experience or a demonstration of supervisory abilities.
  • A minimum of 5 years of experience working in a human services setting.
  • Must pass Criminal Investigation Background and DHHR background checks as well as post‑offer drug screen.
  • Must have a valid driver’s license and current auto insurance.
Essential Duties and Responsibilities
  • Oversee and manage a team of care coordinators and peers, providing guidance, support, and mentorship.
  • Organize and facilitate in‑services/educational opportunities, routine staff meetings, and group supervision.
  • Oversee administrative responsibilities including policies & procedures, compliance standards, HIPAA, incident reports, staff performance reviews, performance improvement plans, hiring assistance, new employee orientation, payroll, etc.
  • Coordinate services and contracts with community partners and/or funding sources, reviewing referrals, documentation, and maintaining budget goals.
  • Manage fiscal operations by monitoring and approving staff billing for contracted services and ensuring productivity standards.
  • Represent Westbrook Health Services in the community and assist with networking and maintaining relationships with collateral contacts.
  • Collaborate with key staff within the company, including the Division Director of Outpatient Services, Outpatient Clinic Director, Executive Team, etc.
  • Responsible for onboarding new clinical staff to a comfortable level with the agency EMR/My Evolv.
  • Demonstrate a willingness to learn and develop new skills, staying updated on current trends, resources, and educational opportunities in the MH/SUD and care coordination arena.
Requirements
  • Excellent interpersonal and communication skills, flexibility, and dependability.
  • Ability to coordinate, analyze, observe, make decisions, and meet deadlines in a detail‑oriented manner.
  • Proficient technology skills and knowledge of Microsoft Suite products, iPhones, and Teams.
  • Capacity to function with minimal supervision.
  • Knowledge of state agencies and community partners.
