Overview
The Primary Care Advanced Practice Provider (APP) delivers comprehensive, patient-centered primary care services within a Certified Community Behavioral Health Clinic (CCBHC) model. This role supports whole-person care by integrating physical health, behavioral health, and substance use disorder services, with an emphasis on access, prevention, chronic disease management, and coordination with interdisciplinary treatment teams. The APP works collaboratively with behavioral health providers, care coordinators, peer support staff, and nursing to ensure high-quality, trauma-informed, and culturally responsive care for individuals with complex medical and behavioral health needs.
Key Responsibilities Clinical Care- Provide comprehensive primary care services, including assessment, diagnosis, treatment, and follow-up for acute and chronic medical conditions.
- Conduct health screenings, physical exams, preventive care, and health risk assessments in accordance with evidence-based guidelines.
- Manage chronic conditions (e.g., diabetes, hypertension, asthma, COPD) with an emphasis on population health and care continuity.
- Prescribe medications and treatments within scope of practice and applicable state and federal regulations.
- Collaborate with behavioral health clinicians, substance use providers, care coordinators, and peer recovery specialists to support integrated treatment planning.
- Participate in interdisciplinary team meetings, case reviews, and care conferences.
- Support warm handoffs and coordinated referrals between primary care, behavioral health, and external providers.
- Complete accurate, timely, and compliant documentation in the electronic health record (EHR) in accordance with CCBHC, Medicaid, CARF, and organizational standards.
- Ensure services meet medical necessity requirements and support audit readiness.
- Adhere to all confidentiality, HIPAA, and data-sharing requirements.
- Support CCBHC quality measures, health outcomes tracking, and performance improvement initiatives.
- Participate in data-driven care planning, follow-up, and outreach for high-risk and underserved populations.
- Promote preventive health, wellness education, and patient self-management.
- Maintain active licensure, certifications, and continuing education requirements.
- Demonstrate professionalism, ethical practice, and commitment to organizational mission and values.
- Participate in training, supervision, and professional development activities as required.
- Comply with federal, state and company policies, procedures and regulations
- Maintain confidentiality of all information and comply with HIPAA regulations.
- Demonstrate knowledge of and support Westbrook Health Services mission, vision, values, policies and procedures, operating instructions, confidentiality standards and code of ethical behavior
Required Qualifications
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in the state of WV.
- Current national certification appropriate to licensure.
- DEA registration and prescriptive authority (or ability to obtain).
- Experience in primary care, family medicine, community health, or integrated care settings.
- Knowledge of chronic disease management and preventive care guidelines.
- Experience working in a CCBHC, FQHC, community health center, or behavioral health setting.
- Experience treating individuals with serious mental illness (SMI), substance use disorders (SUD), or complex social needs.
- Familiarity with trauma-informed care, harm reduction, and recovery-oriented practices.
- Experience with Medicaid populations and value-based care models.
- Strong clinical judgment and decision-making skills.
- Ability to work effectively in an interdisciplinary, team-based environment.
- Excellent communication and patient engagement skills.
- Strong organizational skills and attention to documentation and compliance.
- Ability to manage competing priorities in a fast-paced clinical setting.
- Ability to sit, stand, and move throughout the clinic for extended periods.
- Ability to perform physical examinations and clinical procedures.
- Ability to lift or assist patients as needed in accordance with safety policies.
- Clinical and office-based environment within an integrated behavioral health setting.
- May include exposure to clients experiencing mental health crises or acute medical conditions.
