• Ensures clear and effective communication among healthcare team members.
• Provides timely updates to patients and families about their care and discharge plans.
• Coordinates with various departments to facilitate smooth transitions and avoid delays.
• Develops and implements efficient discharge planning protocols.
• Assesses patients' readiness for discharge and ensures all necessary paperwork and instructions are completed.

The expertise and experiences you’ll need to succeed:

• Bachelor's of Nursing (Required)
• Master's of Nursing
• Basic Life Support - CPR Cert (BLS) - Accredited Issuing Body
• Certified Nurse Manager and Leader (CNML) - EV Accredited Issuing Body
• Registered Nurse (RN) - EV Accredited Issuing Body
