Overview

Responsibilities- Conduct comprehensive patient assessments and develop care plans
- Provide ongoing primary care and education for patients
- Collaborate closely with physicians and interdisciplinary teams
- Build strong relationships with patients and families
- Document patient encounters accurately and timely
- Part-time / Full-time options
Qualified Nurse Practitioner licensed and in good standing. Board certification is preferred. Experience in home-based care or a collaborative, team-oriented environment is a plus.
