Elara Caring is seeking a licensed practical nurse (LPN) to join our hospice team in Paris, TX
.

The Licensed Vocational Nurse (LVN) will provide high‑quality in‑home hospice care to patients and families. You will work under the direction of a Registered Nurse (RN) and physician, following the Plan of Care and ensuring safe, compassionate service.

• Verify the Plan of Care before each visit and provide care per delegation by the RN, physician’s orders, assessment data, and established standards.
• Incorporate patient care goals into nursing visits, documenting on skilled nursing notes.
• Communicate significant findings, problems, or changes in condition to the RN Case Manager, physician, clinical supervisor, or other care personnel.
• Report unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician.
• Implement patient safety plans using available resources.
• Assist the patient in learning self‑care techniques while delivering quality patient care.
• Suggest revisions to the plan of care as needed, notifying the RN Case Manager.
• Provide services that require specialized nursing skills under RN or physician direction.
• Collaborate with the RN Case Manager during normal business hours for advice and instructions.
• Document medication regimen and update medication profile per procedures.
• Treat patients and caregivers with effective, positive communication.
• Implement actions to prevent avoidable hospitalizations and ER visits.

• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• Current State License as a Licensed Vocational / Practical Nurse.
• 1 year of clinical experience in a hospice setting preferred.
• Experience in a home health environment preferred.
• 50% travel required.
• Ability to sit, stand, bend, lift, and move; capable of lifting 50–100 lbs.
• Valid driver’s license, insurance, and reliable transportation.

You will report to the Clinical Team Manager.
