Position: I-CILA QIDP-Bachelor Level position

BASIC DUTIES:

Provides QIDP services to individuals receiving DHS-DDD Waiver Funded Services in HRC’s Intermittent CILA program as well as those individuals in our off-site integrated Tuscola CDS HUB.

Completes QIDP required assessments, implementation strategies and documentation within allotted time frames.

On-call rotation for I-CILA.

Oversees service provision for AHBS individuals ensuring agreed upon services are being provided

Completes monthly summaries and case notes for all Medicaid Waiver individuals assigned

Completes Service Agreements and facilitates/completes Service Authorizations as needed

Completes quarterly home inspections for induvial in the I-CILA program as well as assisting with all medical appointments and linkages.

Assists individual’s in maintaining current benefits including SSI/SSDI as well as Medicare/Medicaid/SNAP

Optional duties including agency’s co-CPR instructor, co-CPI instructor

MINIMUM QUALIFICATIONS:

Bachelor’s Degree in rehabilitation, counseling, psychology, education or related Human services field, at least one year experience working with persons with intellectual/developmental disabilities and meets the definition of a QIDP

Ability to pass CPR/First Aid

Working knowledge of Person Centered Planning processes and QIDP responsibilities

Working knowledge of Home and Community Based Waiver programs and Medicaid Waiver compliance

Ability to pass all background checks

Ability to utilize a computer for case management tasks

Ability to use a phone, computer, copy and fax machine

Working knowledge of Microsoft Word programs

Ability to meet deadlines and ensure completion of required paperwork

Valid driver’s License and ability to drive agency vehicles

DESIRABLE

QUALIFICATIONS:

Knowledge of the habilitation process and principles of behavior management, normalization, and community integration.

Experience in the development of implementation strategies and person centered planning

Ability to work independently.

Demonstrated oral and written communication skills.

Demonstrated ability to work positively and constructively with consumers, their families, and the community.

Knowledge of community day programs for adults with disabilities.

Ability to work as part of a team to implement programs and services.

Job Type: Full-time

Pay: $18.50 - $19.50 per hour
