Nurse Practitioner – Palliative Care (Per Diem)
We are seeking a dedicated Nurse Practitioner to conduct visits to patients on palliative care in accordance with local, state and federal regulations. This per diem position will involve communicating visit findings to the interdisciplinary team, facilitating access to support models of care, and contributing to patient care plans.
Responsibilities- Conduct visits to patients on palliative care in accordance with local, state and federal regulations.
- Communicate visit findings to designated members of the interdisciplinary team, including the National Director of Palliative Care.
- Facilitate access to appropriate support models of care (i.e., hospice, rehabilitation, ongoing aggressive treatment).
- Consult with other members of the interdisciplinary team to contribute to the patient's plan of care.
- Prepare and maintain accurate patient records, charts and documents.
- Assist in the development of clinical practice guidelines/standards in support of quality palliative care as needed.
- Ensure collaboration with patients, caregivers, medical directors and members of IDT.
- Master's Degree in Nursing
- Must have a Nurse Practitioner's license in New Jersey.
- Must have 2-4 years experience working within hospice or related field.
- Palliative care experience preferred.
- Must have a Master's Degree.
- Must have a valid driver's license and a reliable source of transportation.
- Must be willing to travel.
