Responsibilities
  • Social work services, including social work assessment, counseling, consultation, planning, community organization, policy, administration, research, and client-centered advocacy.
  • Clinical social work services under the supervision of a LCSW, including clinical assessment, clinical consultation, psychotherapeutic counseling, and client-centered advocacy. A LSW may not provide clinical supervision.
  • Supervising students who are completing undergraduate or graduate social work coursework in accordance with Council on Social Work Education standards.
  • Provide counseling to clients at both individual and group levels around emotional and mental health issues.
  • Build and maintain strong rapport with clients in both group and individual settings.
  • Provide accurate clinical assessments of clients, including mental and emotional health, stability, and generalized physical assessments for clients with substance use issues.
  • Keep detailed and concise records of each client throughout treatment.
  • Perform professional consultations with medical professionals, therapists, physicians, and, where appropriate, court officers.
  • Follow directions and conduct research in clinical settings under supervision or independently; perform screenings, intake assessments, and consultations for evaluation of individual and family dynamics and treatment needs, including admissions and referrals.
  • Provide ongoing social work intervention and education to patients, families, and significant others for health-related psychosocial problems, based on goals agreed upon by patient, family, significant others, and health care teams.
  • Utilize individual, family, group work and crisis intervention methods to identify and intervene in problems affecting treatment participation and discharge planning.
  • Complete comprehensive psychosocial histories on a timely basis.
  • Provide direct treatment services to patients using modalities including individual, family, and group counseling, and crisis intervention with attention to patient development appropriate to age groups served.
  • Serve as a resource to the health care team regarding chemical dependence, psychiatric and abuse issues; identify, assess, treat, and refer to community services for patients facing child abuse, domestic violence, sexual assault, and elder abuse.
  • Provide case management services for assigned caseloads to ensure treatment plans are followed and to aid patients in obtaining supportive services and resources.
  • Interpret illness and treatment plans to patients and families; communicate family issues to other staff members.
  • Act as liaison to health care team in understanding family dynamics and facilitating timely and proactive discharges to home or another facility.
  • Advocate for patients with other Medical Center programs and liaise with community providers to assist in linkages for patients and families.
  • Make presentations on issues related to the field and available Medical Center resources as part of community outreach to schools, agencies, and other resources.
  • Participate in interdisciplinary diagnostic and treatment planning conferences and make recommendations from a social work perspective.
  • Fulfill all case recording, statistical reporting, and other documentation requirements on an accurate and timely basis.
  • Provide clinical supervision to paraprofessional staff and students as assigned.
  • Provide consultative services to Evergreen Outpatient Program and Medical Center staff and to other community agencies regarding social work issues and services to our patient population.
