• Provides direct social work services in the home which meet Agency and professional standards. Meets Agency standards of competency.
• Accurately documents care provided and maintains patient records according to Agency policies.
• Assists with Agency and community programs/committees.

• Master's Degree in Social Work

• Current Social Work License in the State of New Jersey, LCSW preferred
• 1 year of post graduate experience in a health setting
