As our most acute level of care, our specialty hospitals offer critical care similar to traditional hospitals or ICUs, but for an extended recovery period. Our clinicians play a vital role in the recovery of chronic, critically ill, and medically complex patients.
Job Summary:- The Social Worker Case Manager coordinates, facilitates, and executes social work functions by collaborating effectively with the Interdisciplinary Care Transitions (ICT) team.
- Follows patients throughout the care continuum, identifying and addressing psychosocial needs.
- Provides ongoing support through assessment, treatment plans, case management, mediation, referral, consultation, education, and advocacy.
- Enhances patient management and satisfaction by integrating case management, discharge planning, and social work practices.
- Acts as a patient advocate, investigates adverse occurrences, and performs staff education related to discharge planning and psychosocial healthcare aspects.
- Advocates for understanding physical, biological, psychological, emotional, and environmental factors affecting patient health.
- Coordinates psychosocial activities with the interdisciplinary team for comprehensive discharge planning.
- Uses critical thinking for effective discharge planning.
- Maintains current knowledge of community resources, case management, and psychosocial/legal issues.
- Conducts biopsychosocial assessments for timely discharge planning.
- Serves as a patient advocate, fostering collaborative relationships for informed decision-making.
- Participates in interdisciplinary rounds and conferences.
- Provides education on post-hospital needs.
- Collaborates on care plans and goals with clinical staff.
- Educates patients and families on cultural, ethical, and legal considerations, including patient rights and advance directives.
- Makes appropriate community resource referrals.
- Provides age-appropriate psychosocial support across the lifespan.
- Coordinates social services to support family functioning and healthcare benefits.
- Participates in hospital committees as requested.
- Arranges discharge and post-hospital care through community agencies.
- Graduate of an accredited program;
Master of Social Work preferred. - Social Work License as required by state.
- Case Management certification preferred.
- Three years of healthcare experience; prior case management or discharge planning experience preferred.
- Knowledge of payor practices, regulations, standards, and reimbursement.
- Knowledge of Medicare, insurance processes, and contracts.
- Understanding of accreditation standards and compliance.
