Overview

A regional 282‑bed acute‑care hospital in the Panama City area is seeking an experienced Director of Case Management to lead Case Management, Utilization Management, and Social Work operations.

This high‑impact leadership role oversees departmental strategy, staffing, performance, and care coordination processes within a system that manages 1.2 million patient encounters annually
. The Director ensures effective patient flow, high‑quality transitions of care, regulatory compliance, and optimal resource utilization across the organization.

This position requires a strong, relationship‑driven leader who excels in operational strategy, interdisciplinary collaboration, and driving measurable performance improvement.

Key Responsibilities
  • Provide operational and clinical leadership for Case Management and Social Work services.

  • Oversee care coordination models, discharge planning processes, and utilization review workflows.

  • Manage staffing, onboarding, productivity, and skill mix across all departmental roles.

  • Allocate resources to support patient throughput, quality outcomes, and program goals.

  • Ensure adherence to regulatory and documentation requirements, including CMS and payer guidelines.

  • Analyze departmental and system‑level data to identify performance gaps; implement sustainable improvement strategies.

  • Partner with executive leadership (CMO, CNO, Finance, Legal, Ethics, etc.) to advance organizational priorities.

  • Support financial stewardship through effective utilization management and resource optimization.

  • Build strong relationships with interdisciplinary teams, community partners, and post‑acute providers.

  • Promote a culture of accountability, team engagement, service excellence, and professional development.

  • Participate in budgeting, forecasting, and capital planning for the Case Management department.

  • Provide coaching, feedback, and development for leaders and staff members across the department.

Qualifications
Required
  • 3+ years of recent acute‑care hospital Case Management experience
    , plus 2+ years of leadership experience.

  • Bachelor’s degree.

  • Current licensure as RN
    , LCSW
    , or LMSW
    .

  • Strong knowledge of care coordination, discharge planning, and utilization review.

Preferred
  • Master’s degree in Nursing, Healthcare Administration, Business Administration, or related field.

  • Experience leading Case Management operations in a large, high‑volume hospital
    .

  • Strong communication, analytical, leadership, and change‑management skills.

Compensation & Benefits
  • Salary: $109,762–$164,570/year
