Admissions Coord HCC

Job Purpose

Coordinates the admission/transfer/discharge of patients in accordance with federal, state and local regulations, established procedural guidelines and as directed by your supervisor.

• Participation in the pre-admission and admission process and arranging transfer of patients to other centers when needed.
• Obtaining pertinent information from patients, family members, referral sources and appropriate social agencies, verifying financial information prior to admission and completing admission paperwork with family and patient.
• Verifying that patient rooms and related rooms are ready prior to admission.
• Providing contact and support to patients, families and helping them manage feelings and financial responsibilities.
• Visiting new admissions to facilitate a smooth transition.
• Orienting the patient to the center environment.
• Sending follow-up thank you notes to families within one week of admission.
• Meeting frequently with other social workers and discharge planners within the community.
• Maintaining up-to-date information about referral agencies.
• Maintaining an active waiting list file.
• Communicating admission information to other departments.
• Ensuring understanding of the admission packet and ability to explain it to family and patient.
• Knowledge of long-term care admission requirements, advanced directives and Medicare, Medicaid, Private Pay and Third Party reimbursement.

• Minimum Education
  :
  High school diploma or equivalent, with a minimum of two (2) years of college education in recreation, education, social work, behavioral sciences or clinical services.
• Minimum Experience
  :
  On-the-job training in admission guidelines.
