We are currently looking to fill an Hourly Registered Nurse role at our Southwest Center
.
Shift: Flexible hours within hours of operation.
Salary: The hourly salary for this position is $40.00 per hour. Where candidates fall in this range will be based on skill and experience level.
Essential Job Functions- Meet with clients to review results of lab work that physician ordered as related to the use of psychotropic medication.
- Explain results and answer questions of client and/or family.
- Strategize with client on ways to improve health and wellness such as a change in diet and increase in exercise to improve future lab results.
- Provide psychotropic medication monitoring.
- Monitor and evaluate symptoms of client’s use of psychotropic medications.
- Screen for adverse side effects. Explain possible side effects that clients should be aware of and report to physician.
- Conduct screenings (dyskinesia, glucose, MMSE, or other evaluations to determine if use of currently prescribed psychotropic medicine is causing an adverse effect on the client’s health).
- Provide psychotropic medication training. Meet with client to provide education, technical assistance, and support on the use of psychotropic medications. Provide training to client/family on how to administer medication.
- May facilitate family conferences to identify ways in which family members can offer support in ensuring doctor appointments are made, kept and prescriptions are filled.
- Obtain prior authorization for medication process.
- Perform and document basic patient history, treatments, and administered medications appropriately. Administer prescribed medications (including injections) and treatments as directed by the physician following Agency policies and procedures.
- Obtain vitals (height, weight, blood pressure, glucose) and perform routine functions to prepare patients for examination, and perform TD screenings.
- Maintain accurate medical records and report to the physician any pertinent information regarding patient’s condition and care. Maintain progress notes and medication logs.
- Follow up on lab work and referrals for lab work.
- Maintain medication inventory and sample medications.
- Obtain applications for client assistance.
- Perform individual and small group sessions with clients on topic areas such as diabetic teaching and education regarding heart disease, obesity, food choices/nutrition, and other health risks as it relates to use of psychotropic medications.
- Other duties as assigned.
- Analytical ability necessary to assess a patient’s condition and utilize the nursing process and professional judgment in providing comprehensive care to patients.
- Ability to work effectively with diverse staff and service populations.
- Excellent organizational and time management skills.
- Excellent interpersonal skills necessary to effectively interact with patients, families, physicians, and other health care providers.
- Knowledge of community resources.
- Must be able to communicate effectively, orally and in writing.
- Writing skills necessary to accurately document patient’s assessment and progress in the medical record.
- Demonstrated ability to work in a fast paced environment and meet multiple deadlines simultaneously.
- Bachelor’s degree in Nursing from an accredited school of Nursing.
- Current State of Illinois Registered Nurse license.
- Current CPR Certification issued by American Heart Association.
- 1+ year of Psychiatric Nursing experience.
- Bilingual in English/Spanish preferred.
- APN preferred.
- Satisfactorily pass all relevant screening and criminal background checks.
- Ability to work a flexible schedule including some evenings and/or weekends if needed.
- Client services may be provided in the community.
- Candidates must have regular access to reliable transportation or a reliable vehicle with insurance.
