Job Description

Staff Nurse reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

Responsibilities
  • The registered professional nurse assumes accountability and responsibility for the individualized, age-specific care of a patient along the continuum from admission to discharge. Provides direct care to assigned patients and assists with other patient care as needed. Makes ethical decisions on behalf of the patient/family. Collects and analyzes assessment data to determine nursing diagnoses, collaborates with the patient/family and health care team, develops a plan of care to attain realistic outcomes, provides education based on evidence-based practice, and evaluates the quality and effectiveness of nursing practice.
  • Assess and record patient status and identify care needs upon admission/point of care entry.
  • Organize assessment health data so that it is accurate, complete, and accessible.
  • Identify problems that need immediate attention.
  • Validate diagnoses with patient/family and other health care team members.
  • Document diagnoses to facilitate expected outcomes and plan of care.
  • Initiate a Problem List at admission and update as needed; incorporate age-specific care into the plan of care.
  • Document that the patient’s individualized plan of care has been discussed with and agreed upon by the patient/family.
  • Begin discharge planning at time of admission; consult with patient/family and health care team regarding discharge plan.
  • Identify patient teaching needs on admission and throughout the hospital stay.
  • Arrange conferences with patient and/or family and other health team members to plan care.
  • Prioritize and organize nursing interventions that are evidence-based and align with established standards of care.
  • Provide individualized, mutually agreeable patient and family education based on evidence-based practice and research guidelines.
  • Document all assessments, interventions, and patient responses; review health data to keep the Plan of Care updated.
  • Ensure revisions to the plan of care are systematic, ongoing, and driven by measurable, realistic goals.
  • Document the patient’s progression toward identified outcomes and address unmet outcomes to guide continuity of care.
  • Reassess nursing actions for effectiveness in achieving desired outcomes.
  • Communicate with patient/family and health care team about patient care and nursing’s role in care provision.
  • Collaborate with patient/family and health care team regarding the patient’s progress and plan of care.
  • Present relevant patient information at multidisciplinary meetings/conferences for peer feedback.
  • Make appropriate referrals as needed for continuity of care.
Qualifications
  • Required:
  • Bachelor of Science in Nursing (BSN)
  • Current State of Illinois Registered Professional Nurse
  • American Heart Association Basic Life Support (BLS)
  • Strong interpersonal skills
  • Ability to collaborate with others
  • Strong analytic abilities
  • Associate Degree in Nursing (ADN) acceptable with commitment to obtain a BSN within three years of first day of employment in clinical nurse role
  • Additional requirements for specialized areas (EMERGENCY ROOM, PEDIATRIC EMERGENCY ROOM, NON-INVASIVE CARDIOLOGY) as listed below
  • Emergency Room: ACLS, PALS/ENPC & TNCC within one year of hire
  • Pediatric Emergency Room:
    Pediatric Advanced Life Support (PALS) and TNCC within one year of hire
  • Non-Invasive Cardiology: ACLS
  • Preferred:
    Experience in a specific clinical area
