Key Responsibilities
- Conduct comprehensive health assessments for elderly patients.
- Develop and implement individualized care plans.
- Monitor and manage chronic illnesses and medication.
- Provide education and support to patients and families.
- Coordinate care with interdisciplinary healthcare teams.
- Advocate for the needs and rights of older adults.
- Assist with activities of daily living and mobility.
- Assess and address mental health and cognitive issues.
- Facilitate communication between patients, families, and healthcare providers.
- Stay updated on best practices and changes in geriatric care.
- Completes required organizational compliance education, including assigned requirements that are client‑specified, for Joint Commission Healthcare Staffing Services certification or other regulatory bodies.

- Basic Life Support (BLS) certification.
- A registered nurse (RN) will have a current, full, active license in a State, Territory, or Commonwealth (i.e. Puerto Rico) of the U.S. or in the District of Columbia.
- National Council Licensure Examination for Registered Nurses (NCLEX‑RN) to obtain a nursing license.
- Must possess 2 years of recent experience as a Geriatric RN.
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN).
- Must have completed an accredited training course in nursing through the State of California.
- Work is normally performed in a typical interior/office work environment.
- Work involves sitting and standing for prolonged periods of time.
- May require bending, stooping and lifting up to 15 lbs.
- Must be able to read, write, and speak English to effectively communicate.
- US Citizen or Permanent Resident Card is required.
- Pass/possess VA Public Trust Clearance.
Pay range is based on several factors and may vary in addition to a full range of medical, financial, and/or other benefits. Final salary and offer will be determined by the applicant’s background, experience, skills, internal equity, and alignment with geographical market data.
