A Brief Overview

REQUIREMENT:
Entry level.

The Clinical Nurse Specialist (CNS) is a direct care clinician who also collaborates with members of the health care team to enhance patient care by developing and supporting nursing staff and providing care to patients and families within an identified specialty. The CNS practice competencies are organized within three spheres: patient/client, nursing and nursing practice, and organization/system. The CNS roles include expert clinician, interdisciplinary consultant, educator, researcher, and leader to provide direct patient and family care, shape core competencies of clinical practice, promote evidence-based nursing care, and foster professional growth and system change across SHC practice sites.

The CNS is accountable to a Nursing Administrator at the Director, Nurse Manager, or Clinic Manager level.

This job description covers all CNS classifications within the organization. All CNSs must meet all elements of the Essential Functions, Qualifications, and Supervision. A voluntary Career Ladder exists for promotions based on clinical expertise and contributions to the institution and profession. Promotion is determined per the Center for Advanced Practice Career Ladder Handbook.

What You Will Do
  • The essential functions of the clinical nurse specialist are derived from the CNS Core Competencies delineated by the National Association of Clinical Nurse Specialists (NACNS).
  • Direct Care

    Competency:

    direct interactions with patients, families, and specialty patient populations to promote health and well-being. Conducts advanced comprehensive assessments of patients, families, communities, and social systems using evidence-based techniques. Formulates and prioritizes differential diagnoses and develops evidence-based clinical interventions and systems to achieve outcomes.
  • Recommends therapeutic interventions and treatments; assists staff in developing innovative, cost-effective programs or protocols of care; evaluates nursing practice for patient-centered and evidence-based care; evaluates patient outcomes for safety, timeliness, effectiveness, and efficiency.
  • Other duties as assigned. Additional Direct Care Competencies (privileging required through Medical Staff Office): evaluates and treats patients with acute and chronic health concerns; takes histories and performs physical exams; administers and orders medications; orders and collects specimens; initiates referrals for therapy and nursing services; obtains informed consent; assists with admissions and discharges; recognizes age-specific needs; initiates lifesaving procedures when necessary;

    performs discharge summaries.
  • Systems Leadership: manages change, leads system-wide improvements, develops clinical standards and policies, coordinates care with community resources, and evaluates fiscal implications of practice and system modifications. Participates in and evaluates quality initiatives on Nurse Sensitive Indicators.
  • Consultation: provides specialized expertise and conducts evidence-based practice changes; communicates findings and coordinates patient education and care improvements.
  • Collaboration:

    engages in authentic collaboration to optimize outcomes; builds interdisciplinary teams and coordinates care across inpatient and outpatient settings.
  • Coaching:

    provides education to individuals, families, and specialty populations; promotes professional development of staff and disseminates outcomes of CNS practice.
  • Research: supports evidence-based practice, mentors staff to translate research into practice, and may participate in research within a supportive clinical environment.
