JOB SUMMARY

Provides leadership and management for assigned organ transplant programs. Responsible for the nursing component of the following: management of patient care delivery, human resource management, performance improvement, fiscal management, management of organizational change and operations related to future clinical development and success of the program. The manager will demonstrate a commitment to staff education and development.
ESSENTIAL FUNCTIONS

Must perform all duties and responsibilities in accordance with the hospital s policies and procedures, including its Service Standards and its Code of Conduct.
Management of Patient Care Delivery:
- Assures the delivery of quality, therapeutic and cost-effective care to transplant patients in the inpatient and outpatient settings. Demonstrates the ability to consider age-appropriate needs when interacting with and caring for neonates, infants, pediatric, and adolescent patients.
- Assures that care given to patients is in accordance with standards of practice and desired patient outcomes.
- Determines an appropriate delivery of care model for the Service in cooperation with Patient Care Management, unit staff and physicians. Implements and evaluates safe, therapeutic, cost-effective innovative patient care practices.
- Assures efficient, effective work flow to meet the needs of patients and staff, including adequate number of staff, materials and supplies.
- Assures development, maintenance, accurate interpretation and use of policies and procedures on the unit and within the organization.
- Assures Program s compliance with the state, federal and regulatory mandates (Joint Commission, Title 22, CCS, UNOS, CMS, and California Nurse Practice Act). Develops action plans to ensure on-going compliance with all regulatory mandates. Submits UNOS data and quarterly Medicare cost report surveys on a timely basis.
- Manages work flow within given fiscal parameters. When patient care requirements or other operational needs obligate use of resources beyond monthly portion of annual resource allotment, provides justification to the PCD and implements agreed-upon plan.
- Demonstrates an ability to recognize management problems and collaboratively develops solutions.
- Demonstrates appropriate judgment in referring unresolved and recurrent issues or problems.
- Assists in the provision of patient care when necessary; maintains clinical competence.
- Identifies and/or responds effectively to the services changing needs and helps with priority setting. Anticipates potential problems and formulates plans to prevent them.
- Promotes policies and practices that treat family members as valued members of the health care team.
- Works with team on developing and implementing patient teaching guidelines and checklists.
- Meets regularly with the managers of units that interact with the transplant populations. Ensures compliance with hospital policies and procedures and HIPPA.
Human Resource Management:
- Communicates staff and management performance and expectations. Manages human resources effectively. Provides ongoing and annual performance feedback and evaluation.
- Works with Patient Care Services and Human Resources to attract, recruit and retain qualified applicants.
