Job SummaryThis paragraph summarizes the general nature, level and purpose of the job. As a member of a multidisciplinary health care team, medical social workers provide psychosocial interventions to patients as well as their families receiving services within any department of LPCHS. Work typically addresses a variety of biopsychosocial issues and includes an assessment of the social needs as related to the patient's health status and the collaborative development of a treatment plan, including advocacy within the organization and community, counseling, referrals and practical assistance necessary to accomplish desired objectives.
Provides consultation, orientation, and training to health care staff on the effects of biopsychosocial, cultural, and economic issues on the management of patient/client health needs. Advocates for social work core values and ethics within Stanford Medicine and the larger community.
- Licensed Clinical Social Worker roles: may provide individual/group clinical supervision to MSWs and student interns, as assigned.
- Prepare and maintain electronic medical records in accordance with department and/or health system policies.
- Provide written and oral reports and hand-offs.
- Conduct psychosocial assessments of patients/families upon admission, annually, and/or as clinically indicated to evaluate social, emotional, and physical needs; identify and address barriers to care.
- Collaborate with interdisciplinary health care teams to devise, develop, and implement patient/family psychosocial treatment plans, coordinating referrals to specialty and social services including entitlement programs and other care delivery systems, and recording progress notes and the provision of continuity of care.
- Help patients/families cope with environmental and psychological issues of illness.
- Manage independently assigned caseloads of varying clinical acuity.
- Engage in case finding for patients/families with potential psychosocial needs; attend team conferences and participate in interdisciplinary rounds as needed.
- Provide supportive counseling, grief counseling, crisis intervention, and other therapeutic services to assist patients and families in facing challenges such as coping with illness, processing medical information, and changes in functioning and care needs.
- Provide psychoeducation to patients and families regarding adaptation to the patient s diagnosis, illness, treatment and/or prognosis; assist patients and families in the transition from youth to adult services.
- Participate in quality assurance and community improvement related to social work services or needs of patients and families; contribute to the review and development of written policies, procedures, or guidelines related to social work services.
- Social Work Clinician roles: prepares and maintains electronic medical records; conducts psychosocial assessments; collaborates with care teams to implement treatment plans; helps patients/families cope with illness; manages caseload; participates in team conferences; provides counseling and psychoeducation; transition planning for youth to adult services.
MINIMUM QUALIFICATIONS
Any combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying.

Education:
Master s degree in Social Work from a CSWE-accredited program or equivalent (as demonstrated by licensure).
Experience:
Two (2) years of progressively responsible and directly related work experience.
