Licensed Clinical Social Worker – Blood & Marrow Transplant (BMT)

Stanford Health Care is hiring a Licensed Clinical Social Worker (LCSW) for the Blood & Marrow Transplant (BMT) unit in Palo Alto, CA. This full‑time, 08‑hour day, hybrid position (R2552135) requires a California LCSW license and experience in a medical setting.

As an integral member of a multidisciplinary healthcare team, you will provide therapeutic assessment, intervention, discharge planning, counseling, and referral services to patients, families, and staff. You will address bio‑psychosocial issues, facilitate patient coping, and coordinate care to achieve optimal outcomes.

• Collaborate with interdisciplinary teams to assess and treat social and medical needs of patients, consulting with providers and making referrals.
• Conduct initial assessments of new referrals, evaluate social, emotional, and physical needs using social work theories and techniques; interview patients and families.
• Develop and implement patient management plans, including referrals, progress notes, and continuity of care; provide guidance on coping strategies.
• Manage assigned caseload, maintain accurate records, and prepare written and oral reports documenting case activity, counseling, and therapeutic services.
• Participate in monitoring and evaluating the quality and appropriateness of patient care.
• Provide supportive counseling, grief counseling, crisis intervention, and therapeutic services; educate patients and families about adaptation to diagnosis, treatment, and life changes.

• Master’s degree in Social Work from an accredited college or university.
• Minimum two (2) years of progressively responsible and directly related work experience.
• California LCSW license (Licensed Medical Social Worker).
• Must meet criteria for the Social Worker Professional Development Program.

• Apply medical social work theories, principles, and practices in a hospital setting.
• Gather data to assess patient and family needs.
• Manage caseload, maintain confidentiality, and act appropriately in emergencies.
• Provide written case narratives and reports.
• Use effective assessment, interviewing, and counseling techniques.
• Access hospital and community resources to meet patient care needs.
• Understand patient and family attitudes, behaviors, and needs.

Base Pay Scale:
Generally starting at $55.85 – $74.00 per hour. Final salary is determined by internal equity, experience, education, specialty, and training.
