Continuity Care Home Nurses is actively recruiting for LVNs, with our growing patient base. You will be working in the home providing care to our clientele many of whom have special skills including brain injury, paraplegic patients, ventilator patients, trach patients, etc. We offer great benefits and 401k with match. Qualifications:

• Must have one year of clinical experience, facility or rehab experience preferred
• Licensed LVN or RN
• NO NEW GRADS, please
• CPR Certification - American Red Cross or American Heart Association
• Valid Driver’s License, auto insurance and clean driving record
• Physical/TB test
• 2 work references
• Valid I9 Documentation
• Compassionate and caring demeanor for our patient base

Responsibilities:

• Care for ill, injured, or convalescing patients requiring special skills such as brain injury, paraplegic patients, ventilator patients, trach patients
• Provide basic patient care and treatment
• Collaborate with registered nurses to administer prescribed medications and follow patient’s specific plans of care
• Educate and update patients and families on medical treatments
• Accurate and timely documentation skills are necessary

Wage:

• LVN: $25-35/hr
