Case Manager for Adult Team Services

About the Role

Case Manager for Adult Team Services – Mental Health America of Los Angeles (MHALA) – 35 hours per week, $24 per hour (comp based on experience).

Serves as the primary support and case manager for adults with mental illness by helping them live independently in their chosen community.

• Develop Personal Service Plan and/or Barrier list with each member.
• Mediates and advocates on behalf of the member for housing, health care benefits, entitlements, etc.
• Visits members in residences, hospitals, or jail as needed.
• Transports members to appointments and errands when appropriate.
• Oversees and monitors members’ individual and group activities inside the program and in the community.
• Provides coaching, counseling, mentoring, instruction, training, feedback and positive reinforcement for members in areas such as money management, grooming and hygiene, meal preparation, public transportation and other independent living skills.
• Devises and implements strategies for outreach to members who tend to isolate themselves.
• Knowledgeable in motivational interviewing and harm reduction.
• Supports individuals and team goals in community integration and employment; assists members in achieving community integration and employment goals.
• Participates in staff and community meetings and day‑to‑day operations of the program as needed.
• Required to bill 85 hours per month into the Electronic Health Care System.
• Maintains all records and documentation needed for program compliance and accountability in accordance with MHALA standards and funding source requirements.

• High School Diploma or GED, plus 2 years of experience providing psychosocial rehab related services to a disabled population (preferred).
• All billing and supporting documentation entered into MHALA’s EHR within 24 hours from the date of service delivery.
• All required data collection documents (e.g., MORS, Determinants, KEC, IHOMS, HMIS, etc.) submitted on or before due date.
• Demonstrated knowledge of community resources.
• Works well in a team‑oriented approach to member services.
• Valid CPR certificate.
• Knowledgeable in computer applications such as Microsoft Word, Excel, and Outlook.
• Driving with a current Class C driver’s license; vehicle for transportation, proof of valid automobile insurance, ability to be insured by MHALA liability insurance are conditions of employment.
• Flexible work schedule to include some evenings or weekends, and on‑call cell phone services when necessary.
• TB clearance and re‑testing every year are conditions of employment.
