Physical Therapist Home Health (Palmdale)

Omega Home Health Care Services, Inc. is seeking a compassionate Physical Therapist to provide home health services in Palmdale, CA.

Base pay: $65.00/daily – $140.00/daily

Visit compensation: routine visits $65–$70 per visit, evaluations $80–$100 per visit, admissions $120–$140 per visit.

• Graduate of a physical therapy school approved by the American Physical Therapy Association or the Council of Medication Education of the AMA الأش collaboration with the APTA or the Council of Medical Education of Hospitals for the AMA.
• Licensed or registered by the state of California.
• Preference for at least one year of institutional experience.
• Valid CPR certification, driver’s license, and auto insurance.

• Partner with the physician to evaluate patients through diagnostic and prognostic tests and establish goals.
• Guide patients in therapeutic activities to relieve pain and restore function.
• Record and report patient responses, updates, and changes in condition.
• Train patients, families, and/or caregivers in the use of prosthetic, orthotic, and assistive devices.
• Prepare assessments, plans of care, and incorporate them into the patient record.
• Maintain periodic evaluations and summary reports of patient condition.
• Follow the plan of care and communicate effectively with all care providers.
• Notify the agency of absences, illnesses, or scheduling changes.
• Participate in discharge planning and complete physical therapy discharge summaries.

• Must be able to read 12‑point font or larger.
• Must be able to hear and speak clearly.
• Must be able to stoop, bend, and lift equipment up to 10 pounds.
• Must be able to travel to client residences.
