Overview
Social Worker Case Management LCSW Full Time Nights at Desert Regional Medical Center. The Social Worker is responsible for facilitating care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources, balanced with the patient’s resources and right to self-determination.
Responsibilities- Assess patients for transition needs including identifying and assessing patients at risk for readmission.
- Conduct complex psycho-social assessment and intervention to promote timely throughput, safe discharge and prevent avoidable readmissions.
- Integrate national standards for case management scope of services including transition management, care coordination, and compliance with state and federal regulatory requirements, TJC accreditation standards and Tenet policy.
- Provide education to physicians, patients, families and caregivers.
- Lead a population of patients by service line and/or serve as a resource to Tenet performance standards.
- Complex psycho-social transition planning assessment and reassessment and intervention.
- Care coordination and implementation or oversight of the transition plan.
- Lead and/or facilitate multi-disciplinary patient care conferences including Complex Case Review.
- Make appropriate referrals to other departments and communicate with patients and families about the plan of care.
- Collaborate with physicians, office staff and ancillary departments; assure patient education is completed to support post-acute needs.
- Timely complete and concise documentation in Case Management system; maintain accurate patient demographic and insurance information.
- Precept new staff members and act as a resource to all staff; facilitate TEMPO as needed.
- Participate in department quality improvement initiatives and perform other duties as assigned.
- Experience:
Preferred two (2) years acute hospital experience. - Certifications:
Required:
LCSW based on license requirements of the state in which the Tenet Hospital operates. Preferred:
Accredited Case Manager (ACM). - Sign On Bonus:
Up to $25,000.
Salary range: $35.32 - $54.75 per hour. Individual wages determined based upon qualifications and experience.
