Job Summary

The Masters level Social Worker (MSW) is a member of the Interdisciplinary Team whose primary function is to assess, evaluate, and develop short and long‑term goals with their member/family with minimal direction. Provide diagnostic assessments of the psychological and economic needs of the member/family unit, define social service goals for alleviating these needs, and help provide both the counseling, resources, and casework needed to meet these goals.

Qualifications
  • Master of Social Work Degree (MSW) from a school accredited by the Council on Social Work Education and clinical experience relevant to the psychosocial needs of patients and families. Licensure if Licensed Clinical Social Worker (LCSW).
  • One year of social work experience in a health care setting.
  • Knowledge of stress of repeated loss and capacity to provide support to the rest of the team.
  • Knowledge of community resources and the ability to utilize these resources.
  • Knowledge of the end‑of‑life process and bereavement.
  • Knowledge of non‑invasive pain and symptom control.
  • Demonstrates the ability to use good listening and communication skills.
  • Able to multi‑task effectively.
  • Capacity to relate and work as a member of an IDG.
  • Current California driver’s license and proof of insurance.
  • Travel required.
  • At the discretion of DOHC/FHC management, this position has the potential to be a full or hybrid telecommuting position.
Professional Duties
  • Exercise discretion, use independent judgment and perform thorough assessment of psychosocial, mental health, finances, and support of the family system which could potentially affect coordination of member care (including understanding insurance benefits/coverage, Medi‑Cal, etc.).
  • Assessments will be completed within specified time frames for the Palliative Care and Family Hospice Care programs.
  • Screen for depression, anxiety and other mental illness and assist with facilitation to Mental Health provider as per health plan benefit. In addition, as appropriate, the Palliative Care social worker will establish services with the Behavioral Health clinic per protocol.
  • Determine appropriate interventions needed and communicate with the Interdisciplinary Group to ensure positive member outcomes while coordinating immediate and long‑term goals.
  • Provide psycho‑social and emotional support by acting as a liaison between the member, family, Interdisciplinary Group and any other appropriate parties in the Family Hospice Care/Desert Oasis Healthcare system or community to assure well‑informed health care decisions and continuity of care.
  • Participate in the preparation, evaluation and implementation of the Interdisciplinary Group and develop a plan of care to assure the highest quality of social services is offered to members/families, as well as follow‑up.
  • Educate, coordinate and/or facilitate with the member and/or family any needed community resources.
  • Assist RN Case Manager with member/family education regarding “Life Planning.” This includes completion of POLST, Advance Directive, Serious Illness Discussion, Funeral Plans if applicable and Long Term Care plan. In addition, the Palliative Care social worker will update the Life Planning Template in the EMR and the Hospice social worker will update the entire plan in the EMR and at every IDG.
  • Perform detailed documentation of assessments, follow‑up and coordination of care will be completed in the electronic medical record according to organizational policy.
  • Utilize work time and resources effectively and efficiently, complete assigned tasks within specified time limits while maintaining high productivity.
  • Be available for attending deaths and/or telephone call to the bereaved within 5 days of death.
  • Participate in on‑call rotation.
  • Participate in mandatory and one voluntary in‑service education program.
  • Perform other duties as assigned.
  • Be accountable for your work, speak positively about your work, co‑workers and Desert Oasis Healthcare and Family Hospice Care, see the positive in situations and in others, avoid gossip and negativity, and inform patients and co‑workers of how and when to contact you under normal circumstances (phone number, email, a help line, Immediate Care, etc.) or when unforeseen issues arise.
