Job Summary

Responsible for SNF admissions and concurrent review with all associated responsibilities.

Qualifications
  • High School graduate, preferable with a minimum of a 2 year college education. Current California LVN License required.
  • LVN with 2 years experience in a skilled nursing facility or hospital that involved direct care and interaction with patients and family or discharge planning required.
  • Working knowledge of discharge planning, skilled nursing facility, and home health criteria as well as availability of local social service organizations and community resources/agencies.
  • Familiarity with medical terminology.
  • Must possess excellent telephone, computer and people skills, with an ability to interface with DOHC staff and outpatient providers in a pleasant and professional manner.
  • Must be detail orientated and able to multi-task.
  • Current California drivers license and proof of insurance.
  • Respirator Fit Test (no BLS)
  • At the discretion of DOHC/FHC management, this position has the potential to be a full or hybrid telecommuting position.
Professional Duties
  • Performs concurrent on-site reviews of all local skilled nursing facility admissions.
  • Responsible for coordinating all requests for services not provided in the SNF (i.e. specialist consults, radiology etc.) while maintaining Traxit updates in a timely manner.
  • Attend weekly SNF ICT meetings and provides constructive input towards patient goals along with a timely and appropriate discharge plan of care. Completes SNF skilled and custodial ICT recap report and sends report to SNF Team.
  • Ensures that NOMNC/denial letters are initiated timely and according to Health Plan and regulatory agencies specifics.
  • Contacts admitting and/or consulting physicians directly, when information is necessary to clarify the medical plan of care, level of care, and/or discharge challenges.
  • Keeps Medical Director and/or Designee apprised of SNF activity and needs.
  • Completes discharge screening on all admissions, and coordinates discharge planning needs with the SNF Discharge Coordinator, ensuring timely discharge of medically stable patients, with medically necessary home care and/or allowable Durable Medical Equipment, etc.
  • Accurately completes documentation in Electronic Medical Record (EMR) SNF Template for all SNF cases (skilled and non-skilled) in a timely manner, with completion no later than the discharge date.
  • Emails a SNF Problem Intervention Plan Disposition report on every DOHC SNF members discharge to the appropriate DOHC email groups.
  • Ensures a POLST document is discussed and/or accurately completed with every DOHC SNF admission to include Life Planning Documentation and Serious Illness Discussion Template completion in NG EMR as applicable.
  • Emails the SNF Medical Records packet to the appropriate DOHC email group with every DOHC SNF member discharge.
  • Responsible to complete all due 5 Star Quality Measures that are able to be completed in the SNF setting.
  • Assists the SNF SS Rep to initiate Medi Cal Applications on applicable DOHC SNF admits.
  • Introduce self and your role.

  • Anticipate needs that will arise before your next meeting.

  • Lifting – 0‑40 lbs (equipment, supplies) – Approximately 40% of day.
  • Bending – Approximately 10% of day. Kneeling < 20%.
  • Hearing/Visual Acuity – Adequate for use with computers, telephone, and/or Blackberry – Approx 50% of day.
  • Computer – Highly technical work environment – Must be able to work 7 hours/day using keyboard, mouse, and monitor.
  • Reaching – Above head 75 degrees – Approximately 25%.
  • Hand grip dexterity – Approximately 85% of day.
