Job Description:

* Follows CDC isolation guidelines, demonstrates appropriate use of PPE, performs hand hygiene according to policy, and enforces safety regulations Implements care per the Plan of Care.

* Provides direct patient care to ensure comfort and meet changing needs.

* Educates patients and families on end-of-life care and issues.

* Prepares patients and families for approaching death.

* Promotes patient comfort through appropriate interventions.

Schedule:

 Full Time

Shift: Night Shift 6:45pm-7:15am with every other weekend rotation.

Location:

AH Palm Coast HHA Hospice 150 Memorial Medical Pkwy, Palm Coast, FL 32164 and AH Fish 1055 Saxon Blvd, Orange City, FL 32763 (Covering both facilities but primarily Palm Coast)

Knowledge, Skills, and Abilities:

* Critical thinking [Required]

* Hospice Medicare Conditions of participation [Preferred]

* Critical care nursing [Required]

Education:

* Technical/Vocational School [Required]

Field of Study:

* Nursing

Work Experience:

* 1+ years experience in the health care field [Required]

* Hospice experience [Preferred]

* LTC or Home Health experience [Preferred]
