Additional Locations: AH Fish 1055 Saxon Blvd, Orange City, FL 32763 and AH Palm Coast HHA Hospice 150 Memorial Medical Pkwy, Palm Coast, FL 32164 (primarily Fish)

• Other duties as assigned.
• Manages accurate and thorough documentation per Medicare guidelines.
• Collaborates with healthcare team members to ensure high‑quality patient care.
• Ensures compliance with all local, state, and federal regulations.
• Participates in ongoing professional development and training.

• Associate’s of Nursing (Required)
• Bachelor’s of Nursing
• Basic Life Support – CPR Certificate (BLS)
• RQI Resuscitation Quality Improvement
• Certified Hospice and Palliative Nurse (CHPN)
• Registered Nurse (RN)

Pay Range: $31.53 - $52.24
