Schedule:

Monday – Friday, 10:00 am – 7:00 pm – may travel to other Palm Coast locations at 60 Memorial Medical Parkway.

• Practice principles of employee and patient safety during daily job activities.
• Support consumer marketing and public awareness through participation in community events, educational lectures, and extensions of clinical expertise.
• Implement individualized treatment plans based on physical therapy evaluations, practice standards, insurance guidelines, and patient needs.
• Modify treatment plans as necessary based on ongoing assessments in consultation with the physical therapist.
• Collect patient data and communicate with physical therapists regarding discharge recommendations when maximum benefit has been achieved.
• Document patient care accurately, completely, and timely using measurable and objective terminology, following standards of practice, insurance guidelines, and department/hospital policies.

• Associate’s degree (required)
• Basic Life Support – CPR Cert (BLS) – RQI Resuscitation Quality Improvement
• Physical Therapy Assistant (PTA) – EV Accredited Issuing Body
• Physical Therapy Assistant – TEMP (PTA‑TEMP) – EV Accredited Issuing Body
