Full‑time, days. Variable clinic hours:
Monday‑Thursday 7 am‑7 pm, Friday 7 am‑6 pm.

Primary: 120 Cypress Edge Drive, Palm Coast, FL 32164.
Float Position: 1053 Medical Center Dr, Orange City, FL 32763. Travel also to Victoria Park, Deland Commons, Lake Mary, Daytona, Port Orange, and New Smyrna Beach.

• We serve patients in Orange, Seminole, Osceola, Polk, Volusia, Flagler and Lake Counties with dedicated teams.
• As a Physical Therapist in an outpatient setting, you provide direct and indirect services to specific age groups, utilize process improvement to achieve goals, and practice autonomously and collaboratively.

The Physical Therapist Outpatient professional provides direct and indirect services to specific age groups in the outpatient rehabilitation setting, utilizes process improvement to achieve the goals of the Sports Med and Rehab Department, and accepts responsibility to practice autonomously and collaboratively in all practice environments to provide best practice to the patient/client.

• Engage in promoting evidence‑based standards of care and lifelong learning through autonomous and collaborative participation in research, journal club/reviews, teaching/mentoring/CI, and/or clinical practice education within peer groups.
• Regularly participate in marketing to physicians/physician practices to build referral relationships with a minimum of two contacts per month. Support consumer marketing and public awareness through community events, educational lectures/speaking, and other extension of clinical expertise approved by manager or director.
• Actively support scheduling so that each patient meets the plan of care as agreed upon by the patient and referring physician by adjusting and adapting schedule and/or working within the team to meet patient needs. Willingly adjust to changes in patient census.
• Determine physical therapy diagnosis, establish plan of care, and recommend referral if treatment outside scope required.

• Physical Therapist (PT) Temporary State Permit as Physical Therapist.
• Basic Cardiac Life Support (BCLS).
