RN, Case Manager Responsibilities- Assess and identify patient/family needs, utilize the nursing process, and coordinate the Plan of Care with the Interdisciplinary Team (IDT).
- Provide clinical, palliative and supportive care to the patient/family unit to help keep the participant in their home environment as long as possible.
- Obtain Physician orders and ensure appropriate documentation and communication with IDT.
- Current license as RN in the state where the employee will be working.
- Minimum of one (1) year nursing experience; hospice or hospital experience preferred.
- PACE locations may require certification of Alzheimer’s Disease and Related Dementias Training through the Florida Department of Elder Affairs.
- Previous experience with an EMR/EHR system.
- Mobile Driver – Valid driver’s license and automobile insurance per Company policy; reliable transportation to meet visit schedule.
- Ability to use equipment with visual and auditory mechanisms and to visit participants in their homes for assessments.
- Ability to communicate effectively in English (verbal and written) and perform the essential functions with or without reasonable accommodation (including lifting patients/equipment, bending, pushing/pulling, kneeling).
- Active BLS for healthcare professionals from the American Heart Association or Red Cross.
- Provides reassurance on the phone to patients and families and coordinates in-person visits when needed.
- Uses appropriate resources or personnel to resolve complex situations and documents patient/family information in the EMR and communicates with the IDT.
- Completes initial and semi-annual assessments for Company services, explains services, initiates Plan of Care, and provides skilled nursing interventions to enhance prevention, prevent complications, alleviate symptoms and maximize comfort.
- Represents the Company at assigned facilities and communicates with physicians, facilities, patients, families, and the public; maintains ongoing communication with the IDT.
- Maintains professional relationships with case managers and physicians at facilities.
- Satisfactorily complete competency requirements for this position.
- Represent the Company professionally through care delivered and services provided to all clients.
- Comply with all state, federal and local regulations and Company policies; maintain confidentiality and ethical standards.
- Observe health, safety and security practices; use resources in a fiscally responsible manner.
- Promote the Company through community and professional involvement; participate in quality improvement activities; share expertise with colleagues.
- Provide and manage direct care to patients and families as part of the IDT, incorporating psychosocial, spiritual, cultural, physical and biological components, and appropriate nursing intervention and follow-up.
- Coordinate the Plan of Care to reflect evolving patient needs.
- Educate patient, family and caregivers about disease process, prevention, palliative interventions, caregiving, dying process and safety practices.
- Determine visit frequency based on risk score/needs.
- Conduct home visits to assess home safety, medication, nutrition and DME compliance to enable safe community living.
- Report changes in patient condition to IDT; participate in hospice referral/admission decisions and daily IDT meetings.
- Present concise reports to IDT; maintain effective communication with physicians, staff, patients and families.
- Oversee care by Community Health Workers and Home Health Aides as requested.
