SCOPE OF ROLE

Reporting to the Manager, Nursing‑Primary Care, the Licensed Practical Nurse (LPN), Primary Care will be instrumental in providing outstanding nursing support and assistance to patient treatment plan development. The role will grow and build an excellent patient‑care experience within Primary Care through evaluation and treatment services to patients.

HOW YOU'LL SUCCEED
  • Demonstrate fluency in communicating in English and any required language (e.g., Spanish) verbal and written.
  • Consult and collaborate with staff and providers to correctly execute an appropriate plan of care.
  • Arrange and diligently perform urine toxicology screens, routine lab draws, medication teaching, pill/film counts, observed dosing, medication management, prior authorizations, and refill requests.
  • Obtain vital signs for each patient prior to the provider visit.
  • Provide client education regarding medication and health promotion.
  • Build nursing evaluations and assist in the admission process for new clients.
  • Serve as a subject‑matter expert for patients on calls when answering questions or gathering information.
  • Track and review medical record and lab results.
  • Assist with tracking in the Addiction Service Condition registry.
  • Complete appropriate documentation in the medical record to comply with state, federal, and departmental policies.
  • Coordinate excellent patient care with staff of outside healthcare facilities or emergency rooms when necessary.
  • Create an outstanding reputation when assisting in community outreach.
  • Exhibit a non‑judgmental, empathetic and fully supportive approach when communicating with patients, families, and staff.
  • Ensure quality of care by implementing nursing philosophies and standards of care; enforce adherence to state board of nursing requirements and other governing company regulations.
  • Ensure sensitivity to cultural diversity and maintain age‑related competence for the population served.
  • Monitor operation of medical equipment by verifying availability, completing preventive maintenance requirements, following manufacturer’s instructions, troubleshooting malfunctions, calling for repairs, maintaining inventories, and evaluating new equipment and techniques.
  • Maintain individual clinical and professional knowledge and skill through formal and informal educational experiences.
  • Maintain professional licensure and requirements as designated by the Ohio Board of Nursing.
  • Ask prompted health‑related questions and follow structured screening protocol.
  • Conduct nasopharyngeal and/or anterior nare specimen collection for COVID‑19 screening.
  • Administer vaccines or any necessary medications.
  • Provide patient education regarding COVID symptoms, prevention, testing and when to report to the emergency room.
  • Reconcile lab specimen with the patient’s laboratory order and batch order in preparation for laboratory pick‑up; follow all requirements related to personal protective equipment, hand hygiene, work‑station cleaning and physical distancing.
  • Assist with clinic set‑up, tear‑down and sanitation under the supervision of the Clinic Manager.
  • Participate in quality assurance and utilization review activities.
  • Maintain clear communication with co‑workers, referents or designated liaison person.
  • Comply with all agency policies and procedures.
  • Other duties as assigned.
KNOWLEDGE & EXPERIENCE
  • Completion of an approved program in practical nursing.
  • Valid, unencumbered LPN license in the state of Ohio.
