Full-Time Case Manager for Outpatient Job Summary
The Case Manager provides essential support to clients/residents in their long-term recovery from substance use or co-occurring mental health disorders by facilitating access to health services, coordinating referrals, and assisting with treatment planning. They engage clients/residents in supportive interactions, manage documentation, and collaborate with the treatment team to ensure individualized care. The position requires strong communication skills and the ability to work flexible hours, including evenings and weekends.
Duties & Responsibilities- Assists clients in accessing health, dental, financial, housing, employment, education, and social services based on individualized treatment plans.
- Engages clients in supportive discussions and activities that promote recovery.
- Schedules and provides transportation for medical and mental health appointments as needed.
- Facilitates screening and intake for potential clients and assist with assessments, treatment planning, and counseling as assigned by supervisor, depending on licensure.
- Works with the treatment team to ensure alignment of transition plans with individualized treatment goals.
- Plans and coordinates referrals to support services, monitors clients progress, and advocates for client.
- Acts as a liaison with referral sources and maintains communication to support client needs.
- Manages normal operations and addresses emergency situations independently.
- Upholds professional ethics and client confidentiality, including knowledge of client rights and grievance procedures.
- Completes client charting and required documentation promptly and accurately, including tracking outcomes for quality improvement.
- Attends agency, staff meetings and scheduled in‑services.
- Maintains necessary certification and/or licensure as applicable.
- High school diploma required, undergraduate degree in a human services field preferred.
- Certified Drug and Alcohol Counselor (CDCA) licensure preferred.
- Strong knowledge of substance abuse and co-occurring mental health disorders and available community resources.
- Experience in a residential treatment setting preferred.
- Proficient in computer applications and data management.
- Excellent communication, interpersonal, and organizational skills.
- Availability to work evenings, weekends, and off‑site as scheduled.
- Reliable transportation required.
- Valid Ohio driver’s license and a clean driving record.
- Minimum of 1 year in a personal recovery program, if applicable.
- In this role you may work with or be exposed to individuals experiencing substance use disorders and co-occurring mental health issues. The environment may involve interacting with clients who exhibit a range of behaviors associated with their conditions, including emotional distress, unpredictability, and challenges in communication.
