The post-holder will continue to support the community nursing team but with a wider knowledge and skill set.
You will also support the team in the facilitation of early discharge from hospital where appropriate. The successful applicant will demonstrate a range of clinical skills and competencies along with excellent communication skills providing advice and information to clients and carers with the aim of maintaining them in their own homes enabling maximum independence and optimum quality of life.

About usWe are a large and friendly team where you will have the opportunity to build upon and develop your existing clinical skills. This post requires you to be a team player but confident to work autonomously as required. The locality of Paignton and Brixham covers as far as Kingswear, Stoke Gabriel and Livermead and care delivery will be across this geographical area so the post holder will be required to hold a full driving license and access to a car.
Our service runs from 07:00 - 19:00 7 days a week with various shift patterns and weekend working on a rota basis

PersonSpecification Qualifications and Training
- Foundation Degree (FdSc in Healthcare Practice (Nursing Associate)
- Trust induction training
- Experience in a Community Setting
- Understand and demonstrate evidence-based practice
- Understanding of when to seek advice and refer to a registered care professional
- Understanding of the scope of the role of the Nursing Associate in context of the team and the organisation, and how the role may contribute to service development
- Understand and demonstrate the importance of the promotion of health and wellbeing
- Ability to work independently with a delegated caseload, use initiative whilst understanding limits of scope
- Ability to work effectively as a team player under appropriate supervision, and as part of a multidisciplinary team
- Ability to support the development of less experienced staff
