We are looking for an enthusiastic, experienced nurse or paramedic to join us as a Visiting Clinical Care Practitioner /Community Matron. Because of this success, we are expanding the service to include our housebound patients, giving them better continuity, more personalised support, and high-quality care in the comfort of their own homes.

Main duties of the jobTo deliver a high standard of patient care as a Clinical Care Practitioner in general practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base.
The post-holder will be an experienced nurse/paramedic, who acting within their professional boundaries; will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of their care. They will demonstrate safe, clinical decision‑making and expert care for patient within the general practice.
The post-holder will provide this service to patients in their own homes and at the surgery.
The post-holder will work collaboratively with the multi‑disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
The post-holder will build effective working relationships with other health care professionals.
The post-holder will work within the team of GPs, nurses and health care assistants to deliver excellent primary health care services within the Practice.

The expectation is that the successful candidate will collaborate with our GPs, Nurse Practitioners, Nursing Team and Pharmacists, as well as our colleagues in Community Nursing, Physiotherapy, Occupational Therapy, Social Care and at SWA ST.
