Key Responsibilities- Deliver a high standard of patient care as an Advanced Care Practitioner in general practice, using advanced autonomous clinical skills and a broad, in-depth theoretical knowledge base.
- Provide care for patients from initial history taking, clinical assessment, diagnosis, treatment, and evaluation, both in patients’ homes and at the surgery.
- Work collaboratively with the multi‑disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
- Build effective working relationships with other health professionals, including GPs, nurses, pharmacists, support workers and community services.
- Act within professional boundaries, exercising safe and evidence-based clinical decision-making and, where a physician safe prescriber, prescribing safely and appropriately.
- Prioritise health problems, intervene appropriately including initiation of effective emergency care, health promotion and education.
- Maintain accurate and contemporaneous records using electronic systems, complying with Caldicott Principles, GDPR and NHS policies.
- Participate in clinical supervision, multidisciplinary education and professional development activities.
- Contribute to audit, quality assurance and continuous professional improvement initiatives.
- Support the implementation of risk management, infection control, and patient safety measures.
- Independent Nurse Prescriber
- Mentor/teaching qualification
- Recognised NP qualification at minimum degree level or equivalent
- Clinical supervision training and experience
- MSc Advanced Practice
- MSc Level 7 modules pertaining to Advanced Practice
- Management of patients with acute and long‑term conditions.
- Management of patients with complex needs.
- Clinical examination skills (all systems).
- Accountability of own role.
- Local and national health policy.
- Wider health economy.
- Patient group directions and associated policy.
- Clinical leadership skills.
- Communication skills, both written and verbal.

- Recent primary and community nursing experience.
- Minimum 5 years post registration experience.

- Extended and supplementary nurse prescribing.
- Recent primary and community nursing experience.
- Nurse‑led triage including telephone triage.
- Working with community development initiatives.
- Health‑needs assessment of patients.
- Full UK driving licence.
- Self‑directed practitioner.
- Highly motivated.

- Ability to work across boundaries.
