Social Services Director

Job Duties
  • Conduct comprehensive assessments of residents' psychosocial needs and develop personalized care plans in collaboration with the interdisciplinary team.
  • Provide emotional support, counseling, and resources to residents and their families to address their social and emotional well‑being.
  • Assist residents in accessing community resources and services to meet their individual needs and preferences.
  • Facilitate family meetings and serve as a liaison between residents, families, and the facility to ensure open communication and understanding.
  • Collaborate with medical professionals, including doctors, therapists, and social workers, to ensure the holistic care of each resident.
  • Assist residents with discharge planning and coordinate the transition to home or other care settings when necessary.
  • Maintain accurate and confidential resident records, ensuring compliance with all relevant regulations and policies.
  • Stay updated with changes in regulations and best practices related to social services to enhance the quality of care provided.
Qualifications of the Social Services Director
  • Minimum of a Bachelor's degree in Social Work or a bachelor's degree in a human services field, including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology.
  • One year of supervised social work experience in a health care setting, working directly with individuals.
  • Strong communication and interpersonal skills to interact with residents, families, and staff effectively.
  • Empathetic and caring nature, with a genuine desire to improve the lives of our residents.
  • Knowledge of relevant regulations and laws related to social services in a healthcare environment.
  • Ability to work collaboratively with a diverse team and maintain a positive working environment.
